Congenital Absence of the Gall Badder and Cystic Duct



Abstract
Three cases of ageneiss of the gall bladder and cystic duct is reported. The diagnosis of the agenesis of gall bladder should rely on the findings of operative cholangiogram and operative findings. It should rule out the following conditions.
a) Contracted gall bladder
b) Intra-hepatic gall bladder
c) Ectopic gall bladder
The pathogenesis of the dilatation of the common duct associated with agenesis is obscure. This condition was found in one of three cases. In this case, pancreatitis was associated with the pathology. The rest of two cases had intra-hepatic stones and unusual elongation of left hepatic duct.
A commonly mentioned theory ascribes the dilatation of the common duct to its role in taking over the storage function of the absent gall bladder, or in another theory to obstruction from fibrosis of the Ampulla of Vater. None of these theories are verified yet.
Most of cases in the agenesis of gallbbladder those who did not have biliary stones, but suffered from biliary colic, has been relived by the C.B.D. drainage.
Whether agenesis of the gall bladder in the absence of common duct stones is capable of producing symptoms of biliary disease is obscure. Likewise the treatment in such cases is quite vulnerabe.
In our experience, it must be judged by pre-operative and operative findings.
Further follow-up study should be made to clarify the operative results and pathogenesis of associated diseases.
